The answers to that are training, your buddy, and redundancy.
Divers are trained how to handle gear failures such as gas supply, mask, and so forth. And a good diver knows when to call the dive before problems have a chance to escalate.
Your buddy should be there if you need him, and know how to assist by sharing air, etc.
Technical divers have led the way with redundant gear configurations: dual regs, spare mask, backup lights, whatever.
Gear failures can happen at any time, some types more than others. Proper training and procedures keep small problems from becoming big ones. Murphy's law rules: if it can go wrong, eventually it will.
